The Major Seven Elements Of a Model Of Prescription Are Follows :-


  1. PATIENT INFORMATION : - Name,address ,age of the patient are necessary so as to avoid wrong delivery of medication to any other patient.Age and weight of the patient help in the dose of the prescribed drugs, especially in the case of pediatric and geriatric patients.
  2. SUPERSCRIPTION : - The symbol 'Rx' which is the abbreviation of Latin word 'recipe' meaning- 'take thou'.Initially it was believed to be the sign of Jupiter(♃) ,THE GOD OF HEALING.
  3. INSCRIPTION :-The main part or the body of the prescription which contain the names and quantities of each ingredient require to compound and dispense products.A physician may ask the pharmacists to dispense an official preparation without giving its formula in detail.
  4. SUBSCRIPTION : - It includes directions to the pharmacists by the physician regarding the dosage form and the number of to be compounded and dispensed.
  5. SIGNATURE (SIGNA) : -The part of prescription which consists of direction to the patient,regarding the dos,route of administration,frequency and time of administration.If prescriber writes the word 'LABEL' in the signa, it is instruction to the pharmacists to mention the name and strength of the drug on the label.
  6. REFILL INSTRUCTION : -It consists of the direction to the pharmacists regarding repetition of the prescription for chronic treatment.It avoids misuse of medicine by the patient.The direction may be given as follows :
  • "repetature" - 1 2 (3) 4 5 : repeat up to designated number.
  • "p.r.n'(pro Renata) - repeat when occasion arises.
  • "n.r." (non repetature) - do not refill.
